Organ Transplantation Project/NGO + LaTeX survey/questionnaire script

Here is a survey script (courtesy of: svenhartenstein.de). I've been working with a group of physicians setting up a start-up NGO to raise awareness and research regarding organ transplantation in Egypt. This issue has societal and religious taboo, so we are treading lightly for the moment. Yet I believe this survey script may be of some use for others.
